What We Do
- Assess and work with the family to stabilize any immediate crisis
- Coordinate a service planning process with the family and any other relevant agencies or professionals
- Ensure that services to the child and family will be delivered and coordinated by the appropriate community agency
- Arrange a face-to-face meeting with the family and young person, as soon as possible
- Provide brief family-focused interventions to support the youth/family around the issues that led to the referral
- Coordinate access to the services and resources identified in the planning process
- Maintain communication with designated KHCAS and DBCFS contacts
Who We Serve
- Families of children and youth from 10 to 17 years of age, where the child / youth is experiencing significant to severe mental health and behavioural problems that threaten their continued placement in the family / caregiver home.
- Referrals for this service may only be made by KHCAS staff and DBCFS personell

- Reduce the number of youth going into care
- Achieve concrete goals negotiated with families
